Unrelated in just random information the Surgeon General isn’t a uniformed general.
He’s a Vice Admiral and the operational head of the PHSCC, one of the uniformed services of the US.

If the Asst. Sec of Health is a commissioned officer in the PHSCC, they would outrank and be a full admiral. If the Asst Sec is not, the SG would be the highest-ranking member of the PHSCC.
