@wp_smith @carlhancock mine was after a recent update too. Just the one of seven forms failed. Short contact form.

@wp_smith My case was an existing form that was working just started displaying that. Recycled the page and it worked. Couldn’t duplicate.

@wp_smith Did you get that on a site for no apparent reason? I just did. Deleted the shortcode and readding it via the modal worked for me.
